Need a New Account?Just follow these easy steps! 1. Read and agree with our Payment and Credit Policy for Services 2. Payment and Credit Policy for Services Payment with specimen is appreciated. We accept checks, money orders, Visa/MasterCard/Discover. We do not accept American Express. Please note that patients ordering tests directly are subject to a surcharge of $20 to $40. Current account billings are possible if signed and indicated by the ordering healthcare provider. We require payment within 30 days. If an account becomes delinquent, we reserve the right to put your account on hold until payments are received and your account is brought current. Our maximum credit line is $2,500.00 per client. In appreciation of your business, we currently offer a 50% discount on the first test submitted under a new healthcare provider account. The healthcare provider may choose to apply this rebate to him/herself, or to a patient. The discount must be indicated at the top of the requisition form. We will not honor discounts after the requisition form has been submitted for processing. No Payment, No Test Account Healthcare providers: If you prefer to not have your account billed, you can opt for a "No Payment, No Test" billing status. Patient must send a money order or cashier's check with the specimen. If payment is not included, testing will be delayed, regardless of specimen integrity. Please ensure that your patient understands this in order to prevent the hassle of repeat collections. To establish a "No Payment, No Test" account, please fax or mail us an endorsed authorization for the NPNT billing status to be placed on your account. Insurance We are not a Medicare certified facility and do not accept Medicare assignment as payment for lab testing. We are not certified to process Insurance claims for reimbursement. We will provide you or your patients with invoices and CPT codes if you want to file a claim. Please indicate on your requisition form if you desire a receipt to be sent out. Because we do not treat your patients, we are unable to provide Diagnosis codes for Insurance providers. Important! Please have your staff take a moment to enter all the data on the requisition form, including ADDRESS, PHONE NUMBERS, and FAX NUMBERS for both the healthcare provider and patient. Also, requisitions without a healthcare provider's signature will not be processed until that information is provided. If you are the referring healthcare provider but another agency is financially responsible for payment, please complete all information for yourself and write 'Bill to' and that information along side. We solicit your assistance in improving our customer service by not giving out our phone number to your patients. Many of our Clients (healthcare providers) are referring their patients to us for interpretations and/or medical advice that we are unable to provide. When technical or medical questions are asked, we must refer the patient back to you, the healthcare provider who is treating the patient. Patients requesting technical information severly impact laboratory productivity. We thank you for your understanding. Interpretations of lab results are available to healthcare providers at a cost of $35.00, and healthcare providers may schedule a 15-minute phone appointment. Please call for phone appointment fees. Our business hours are Monday through Friday from 7:00am to 5:30pm Pacific Time.
