For practitioners and patients returning kits to MVL.

If your kit already includes a pre-printed return label, you can use that label and skip this step.

Before you start

  • Have your completed kit(s) sealed and ready to ship.
  • Use the original kit box or packaging when possible.
  • Confirm your return address and contact details.
  • Inside the UPS tool, choose “View and Print” for label delivery.
If you ever land on a generic access screen, it usually means the Company value was not passed through. Re-check that the hidden field is still present in this block.
What You’ll See

How the Returns Tool Works

The UPS form will ask for basic “Ship From” details and your package information.

  1. Enter your “Ship From” information (name, address, phone, email).
  2. Select your country and state as prompted.
  3. Choose a Label Delivery Method and select “View and Print.”
  4. Select the Sample Type that matches your shipment.
  5. Add your package if required by the form.
  6. Click “Process Shipment” to generate your label.
  7. Print and attach the label to your sealed package.
  8. Drop off at UPS or schedule a pickup if available.

If the UPS tool errors, doesn’t recognize your sample type, or you are returning from outside the U.S., contact MVL Client Services for the best next step.

  • Can I return more than one kit in a single shipment?
    Often yes, if kits are securely packaged. When in doubt, ship separately or check with Client Services.
